800-786-4751. In order to receive prescription medications from Revival, we will need
the prescription from your veterinarian. We can accept prescriptions through these

Fax
Your veterinarian can fax the original prescription from their office to our pharmacy at 877-335-2680. We can only accept faxed prescriptions from a veterinarian, so please do not fax the prescription yourself.

Your veterinarian can call our pharmacy directly at 712-737-5555 x134 and authorize the prescription. If our Pharmacists are not available, please leave a message. All prescription authorizations will require an order and payment information before we can ship the medications. If your veterinarian has sent in a prescription form, please place an order online or over the phone.

A prescription may be transmitted by facsimile (fax) to the
pharmacy only by the prescribing veterinarian. The prescription can not be
faxed to the pharmacy by the owner of the animal. The owner may obtain the original
prescription from the vet and mail it in.
Federal (U.S.A.) law restricts the
use of Rx drugs to be used by, or on the order of, a licensed Veterinarian.
Please Note: Due to federal and state law, we cannot accept returns of
prescription medications, so please make your selections carefully.
What is the Puppy-DPv vaccine?
Keep your puppies safe from common canine diseases like parvovirus and distemper by giving them the Nobivac® Puppy-DPv vaccine. This vaccine is perfect for healthy dogs ages 4 weeks or older, as it covers all known strains of parvovirus and protects against other diseases due to distemper or parvovirus. The puppy-DPv vaccine uses modified live versions of the viruses to safely introduce them to puppies and help them build immunity.
This vaccine requires reconstitution using the diluent included in the packaging. Also, you should use new, nonchemically sterilized needles and syringes. Effectively protect your puppies from distemper and parvovirus by using the best practices possible when administering the vaccine.
Puppies younger than 12 weeks old should receive the initial dosage and have additional booster shots every three to four weeks until they reach 12 weeks. If administering the DPv vaccine to dogs older than 12 weeks, you should give them two doses, three to four weeks apart. Annual one-dose revaccination is recommended, but discuss if it is necessary with your veterinarian first.

Avoid vaccinating pregnant dogs.
Do not freeze the vaccine.
Store in areas with temperatures ranging from 35 degrees Fahrenheit to 45 degrees Fahrenheit.
Use the contents directly after reconstitution.
Burn all unused contents and containers to dispose of them properly.
- Vaccinate healthy puppies 4 weeks of age or older.
- Aseptically reconstitute the desiccated vaccine with the sterile diluent provided.
- Administer 1 ml subcutaneously or intramuscularly.
- Initial vaccination of healthy dogs may be given as early as 4 weeks of age, with booster injections administered every 3-4 weeks until the dog is 12 weeks of age.
- Dogs over 12 weeks of age should initially receive 2 doses 3-4 weeks apart.
- Annual revaccination with one dose is recommended.
